REQUIREMENT FOR CHILDREN AND YOUTH SACRAMENT PREPARATION
Sacrament Preparartion is 2 consecutive years of preparation. Your child is eligible to sign up for the Sacrament Preparation class after your child has already completed one full year of Faith Formation the previous year. If Faith Formation was completed from another parish, we need for you to provide a letter from the director/coordinator of that parish before we can process your registration form. You will also need to supply us a copy of the child's baptismal certificate before we can process your registration form. A completed Confirmation Sponsor form is also required before class begins on September 26, 2021.
If this is your first year of Faith Formation, use the online Faith Formation Registration form to join our program. The kindergarten program is NOT considered as the first year of preparation for Sacrament preparation.
If 2021-2022 is your child's second year of Faith Formation, do join our Sacramental Preparation program -- Fill this form AND the Faith Formation online form -- online Faith Formation Registration. Do note that your child will continue to be a part of the Faith Formation program while preparing to receive the Sacraments therefore, submit both forms.
Note: In joining our program its a parent's commitment to grow their children in the Catholic Faith. Hence, you are agreeing that your child will continue to be a part of the formation program even after receiving the Sacraments. Holy Family is a " restored order" parish wherein the celebration of the sacraments of Confirmation and First Eucharist are received together. “Restored Order” is to celebrating the sacraments of initiation in the order in which God designed them to be administered: Baptism, Confirmation, and First Eucharist. It also restores the practice of administering the sacraments of Confirmation and First Eucharist to children of early age, which is how the early Church practiced the first 500 years.
